ROOTS Community Integration

ROOTS Community Integration and Job Preparation provides a well-structured program that emphasizes meaningful activities in the local communities, academic advancement, vocational training, and an avenue for using special talents and abilities. The goal is to increase autonomy and self-reliance, empower choices, maximize dignity, and encourage positive social interactions and healthy lifestyles.


Admission Criteria for ROOTS Community Integration and Job Preparation

Persons applying must meet the following criteria:

  • Diagnosed with a developmental disability prior to the age of 18
  • Have been eligible for a free, public education
  • Demonstrates basic communication skills
  • Motivated to learn skills to increase independence
  • Possess sufficient emotional maturity and stability to participate in all aspects of the program
  • Demonstrate the ability to accept and follow reasonable rules and expectations
  • Interact respectfully with peers and staff
  • Capable of maintaining behavior which does not impede oneself or others from fully participating in all aspects of community activities
  • Does not pose a danger to oneself or others

The Farmer’s House is open to applicants meeting the above criteria regardless of socioeconomic status, race, gender, sexual orientation, religion or national origin.

Once a person interested in applying for The ROOTS Community Integration and Job Preparation Program has reviewed the ROOTS Community Integration and Job Preparation Handbook in conjunction with the Admission Criteria an Initial Intent Form needs to be completed.  If you require a printed copy or have any questions concering ROOTS, please contact Kelly Cogan, This email address is being protected from spambots.
